You are describing and detailing a broad systemic problem that can only be addressed by open, meaningful discussion, awareness and commitment. That will NEVER happen under GOP leadership. The GOP is unwilling to acknowledge and certify the most open and honest presidential election in our history. An election that garnered the most votes ever cast! I will not live long enough to see the fundamental change required happen. The future, and much of the present, belongs to the young people who care.
I love this 'last mile' description. I always thought that prioritizing areas by zip codes and taking the vaccine directly there would help address the access problem while also reaching the groups that are most affected.
If people don't/won't get vaccinated, that is their choice. Vaccinations are available to almost everyone, free. Time to open up. We shouldn't suffer for the non-believers. Schools need to open fully. I'm a democrat, and it is time for this to come to and end. Can't cure the stupid.

Access is key. Using the last mile metaphor is inspired. This is a concept that a variety of disciplines understand.
